Assist Life Stage/ Transition

Transitions like moving out of home, starting university, or getting a job are significant events in everyone’s life. These changes can be challenging both emotionally and practically, especially for people with disabilities. The new environments and settings add to the difficulty.

The National Disability Insurance Scheme (NDIS) provides various supports to help with life stage transitions, aiming to make these changes as smooth as possible without disrupting the person’s comfort. Disability support agencies offer short-term and long-term support options and life transition planning services based on individual needs. The goal is to ensure a smooth transition, helping participants settle into new environments and feel comfortable with accommodations, social connections, finances, and recreation.
Centre of Care Services’ Life Stage Transition program offers short- and long-term support focused on living at home, participating in the community, and coordinating your support. This includes:

  • Support connection and coordination
  • Capacity building skills
  • Development of independent self-care and living skills
  • Crisis and conflict resolution
  • Establish supportive networks in the participant’s communal setup
  • Linkage support to other supportive community services and programs
  • Assistance with accommodation, tenancy procedures
  • Daily planning and budgeting
  • Mentoring and peer support
  • Individual skill development
  • Assistance with decision making and planning
